A-Vision’s Community Health Worker (CHW) Services Division delivers flexible, community-based solutions that support outreach, education, navigation, and workforce development. Our services are designed to meet people where they are—culturally, geographically, and relationally—by embedding trusted, trained Community Health Workers within the communities they serve. We partner with organizations to extend reach, strengthen engagement, and improve access to education and supportive resources.

o Contract CHW Staffing

Deployment of trained Community Health Workers to support outreach, engagement, care navigation, and community-based initiatives. Staffing solutions are scalable and outcomes-oriented, helping organizations expand reach, improve participation, and strengthen continuity of support.

o Wellness Visits, Outreach & Navigation

Community-based and in-home wellness visits that deliver health education, resource navigation, and referrals to trusted services. CHWs focus on relationship-building, follow-up, and reducing access barriers through consistent, culturally responsive engagement.

o Community & Faith-Based Program Facilitation

Facilitation of culturally relevant health education and engagement programs in trusted community settings, including churches, housing communities, and nonprofit spaces. Programming supports participation, trust-building, and sustained connection while creating pathways for community leadership and workforce development.
